As children grow, their oral development plays a big role in how they eat, speak, and even breathe. One often overlooked issue involves the small bands of tissue under the tongue or upper lip, known as the frenum. When this tissue is too tight or restrictive, it can limit movement and cause a variety of functional challenges. This condition is commonly referred to as a tongue-tie or lip-tie, and in some cases, a frenectomy may be recommended to correct it.
A frenectomy is a simple procedure that releases the restrictive tissue to improve mobility and function. Because symptoms can sometimes be subtle, many cases go undiagnosed early on. Here are five signs that your child may benefit from an evaluation.
1. Difficulty Feeding in Infancy or Early Childhood
One of the earliest signs of a restrictive frenulum can appear during feeding. Infants with tongue-tie may struggle to latch properly during breastfeeding or bottle-feeding. This can lead to prolonged feeding times, frustration during meals, or inadequate weight gain.
In older children, feeding issues may present as difficulty chewing certain textures or frequent gagging. If your child consistently struggles with feeding milestones, it may be worth evaluating whether restricted oral tissue is playing a role.
2. Speech Delays or Pronunciation Challenges
Clear speech requires a full range of tongue movement. When the tongue is restricted, children may have trouble forming certain sounds, particularly those that require elevation or extension of the tongue, such as “t,” “d,” “l,” or “r.”
While speech development varies by child, persistent difficulty with articulation despite practice or speech therapy support may be a sign that physical restriction is contributing to the issue.
3. Limited Tongue or Lip Movement
A key indicator of a tongue-tie or lip-tie is restricted movement. Your child may have difficulty sticking out their tongue, moving it side to side, or lifting it toward the roof of the mouth. In some cases, the tongue may appear heart-shaped when extended.
For lip-ties, you may notice difficulty flanging the upper lip outward, which can also affect oral hygiene and feeding efficiency.
4. Ongoing Oral Hygiene Challenges
Restricted oral tissue can make it harder for children to clean their teeth effectively. Limited tongue movement may prevent natural clearing of food debris, increasing the risk of plaque buildup or cavities.
Parents may also notice difficulty brushing certain areas of the mouth or persistent food trapping along the gums. Over time, this can contribute to dental health concerns if not addressed.
5. Sleep or Breathing Concerns
In some children, tongue or lip restrictions may contribute to mouth breathing, snoring, or restless sleep. Proper tongue posture helps support healthy airway function, and restrictions can sometimes interfere with this natural positioning.
If your child frequently sleeps with their mouth open or shows signs of disrupted sleep, an oral tissue evaluation may be helpful in determining whether a frenectomy could improve function.
Why Early Evaluation Matters
Identifying oral restrictions early can make a significant difference in a child’s development. Treatment, when needed, is typically quick and well-tolerated, and may help improve feeding, speech clarity, oral hygiene, and overall comfort.
If you notice any of these signs, a pediatric dental evaluation can help determine whether intervention is necessary and what the best next steps may be for your child’s needs. Small issues in oral development can have a big impact over time, so early awareness is key.
